You can find one copy of Leftovers in the basement deck of the S.S. Tidal. But you can only board this ferry if you have the S.S. Ticket, which you only get in the post-game.
If you want to get Leftovers before the post-game then your only option is to use the Pickup ability. Any Pokémon at Level 81 or higher with the Pickup ability has a 1% chance of picking up Leftovers after every battle — even if the Pickup Pokémon didn’t participate in the fight.
But since Pickup doesn’t activate 100% of the time, you have a less-than-1% chance of actually obtaining Leftovers this way with each battle.
Method #1: S.S. Tidal (post-game)
You’ll first need to finish the Delta Episode before attempting to reach this Leftovers’ location. To board the S.S. Tidal, you’ll need the S.S. Ticket. This will be given to you by Gym Leader Norman in the post-game.
You can use this ticket to board the S.S. Tidal in the harbors of Slateport City or Lilycove City.
Once you have an S.S. Ticket, here’s how you can get on the ship:
Step 1: Fly to either Slateport City or Lilycove City and go to the harbor.
You can find the Slateport City Harbor on the northeast corner of town, and the Lilycove City Harbor on the southwest corner of town.
Step 2: Speak to the woman inside the harbor and show her your S.S. Ticket.
Then you’ll have to travel to one of the other cities. If you’re in Slateport City, choose Lilycove City as your destination, and vice versa.
Step 3: Once you’re aboard the ship, take the stairs directly down to the bottom deck.
Step 4: Once you’re below deck, you’ll find a couple of Sailor NPCs that will battle you if they see you.
But from the staircase just go to the northwest corner of this room – you’ll find the Leftovers there behind some red storage crates.
Method #2: Pickup (Ability)
A Pokémon with the Pickup ability has a 10% chance of getting a random held item after each battle. The item you get is pulled via RNG from a pool of 11 possible items.
At Level 81 or higher, Leftovers become a part of the item pool with a 1% probability.
While this method is repeatable, there’s an extremely low chance of obtaining Leftovers this way, so it is generally not recommended unless you really want more than 1 Leftovers in your save file.
Uses
A Pokémon holding Leftovers will heal for 1/16th of its maximum HP at the end of every turn. This item is generally beneficial to any Pokémon, so it’s one of the better held items in the game.
To make the most use of this item, give it to stall Pokémon such as Cresselia and Dusknoir. They have high base HP, Defense, and Special Defense stats. And with their slow playstyles they can benefit from regaining a bit of HP every turn.
